Your trust matters to us. Here's how we protect your information and what you can expect as a Haven Health patient.
Notice of privacy practices
Haven Health is required by law to protect the privacy of your health information, give you notice of our legal duties and privacy practices, and follow the terms of our current Notice of Privacy Practices.
We use and share your health information to treat you, run our practice, and bill for services. We will not use or share your information for other purposes, such as marketing, without your written permission.
You have the right to:
- See and get a copy of your health records
- Ask us to correct your health records
- Request confidential communications
- Ask us to limit what we use or share
- Get a list of those with whom we've shared your information
- Get a copy of our privacy notice
- File a complaint if you feel your rights have been violated

Confidentiality of substance use treatment records
Records of substance use disorder treatment are protected by an additional federal law, 42 CFR Part 2, on top of HIPAA. In general, we may not say to anyone outside the practice that you are a patient in substance use treatment, or disclose any information identifying you as such, without your written consent, unless the law expressly allows it.
Violations of these protections may be reported to the appropriate authorities as described in our full notice.
Patient rights & responsibilities
As a patient, you have the right to:
- Be treated with dignity, respect, and without judgment
- Receive clear information about your health and treatment options
- Take part in decisions about your care, including the right to refuse treatment
- Privacy and confidentiality
- Voice concerns or complaints without fear of retaliation
We ask that you:
- Share accurate and complete information about your health
- Follow the care plan you and your provider agree on, or let us know if you can't
- Treat staff and other patients with respect
- Keep appointments or let us know when you need to cancel
Nondiscrimination notice
Haven Health complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex (including sexual orientation and gender identity). Haven Health does not exclude people or treat them differently because of these characteristics.
If you believe we have failed to provide these services or discriminated in another way, you can file a grievance with our office by calling (443) 717-2302. You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights.
